We have found that on day 21, the offspring of the pregnant rats having their uteri removed have elevated levels of mucosal inflammatory cytokines and acute phase proteins and decreased levels of antioxidants. This systemic inflammatory response in the absence of maternal nutrient availability is similar to that observed in human fetuses in growth-restricted pregnancies. Furthermore, preliminary data from in vitro studies show that exposure of cultured human trophoblasts to inflammatory cytokines results in reduced cell growth and altered function of the placenta. The current application proposes to further explore the role of inflammation in growth restriction. Our hypothesis is that the inflammatory response of the immature fetus in the face of nutrient restriction is responsible for the high mortality in this group and that treatment of the fetus with anti-inflammatory agents can normalize the growth restriction. The specific aims of this proposal are to: 1. determine if inflammatory mediators are elevated in the intestine and blood of fetuses with growth restriction; 2. assess if anti-inflammatory agents can ameliorate the growth restriction; 3. determine if inflammatory mediators can be detected in intestinal explants from fetuses with growth restriction; 4. determine if inflammatory mediators can be detected in umbilical blood from growth-restricted fetuses; and 5. determine if inflammatory mediators can be detected in fetal plasma in the presence or absence of maternal nutrient restriction.
